In ChangeRequest, two of the most important and commonly used forms are the case and project forms. They are used mainly to obtain useful information either from a Client, an Analyst or a PM. We deal with these forms every time we add a new case or project.
Through the Case and Project Fields feature, Project Managers can customize case and project forms by adding new fields (examples shown in the table below). This is particularly useful when you want to capture data other than what is asked in the present forms.

To add a new field to the case/project form, do the following:
- Go to Settings > System Settings.
- Click on either the Case Fields or Project Fields tabs depending on where you want to add the field.
- Click on the Add another Field link.
- Select the Field Type in the list provided.
- In the Field Name box, type the caption/label that will give the users an idea on what type of input they should supply in the field (e.g., Project Name)
- Check the Required option if you want to oblige the users to fill out the field.
- If you want to provide some kind of instructions or examples to the users to help them understand what the field is for, type a Text Explanation in the box provided.
- (For Radio Button, Checkbox, Dropdown List, and Multiple from a List) In the Label box, type the caption/label that you want to appear as an option in the field. For example, if you create a Radio Button field and name it “Project Type”, you may want to create two options with Labels “Internal” and “External”.
- (For Radio Button, Checkbox, Dropdown List, and Multiple from a List) In the Value box opposite the Label box, type the caption/label that will be stored in the database when the option is selected by the users. It is a good practice to have the same entries for both Labels and Values boxes. Just make sure that Value is in one word only. For example, if you have an option with Label “Small Enterprise” for your field “Company Type”, you should use “SmallEnterprise” as the Value for the option.
- (For Radio Button, Checkbox, Dropdown List, and Multiple from a List) If you want to add another option for the field that you are creating, click on the Add another option link.
- Click on the Save Changes button. After doing this, the field will immediately become available in the case/project form.
